How long is ACL rehab, and when can I return to sport?

After ACL reconstruction, most athletes need about 9 to 12 months of rehabilitation before returning to cutting and pivoting sports, and some need longer. The date matters less than meeting strength, movement and confidence criteria, and your surgeon makes the final call on clearance.

Not every ACL injury is treated with surgery. Some people rehabilitate without it, depending on their knee, their sport and their goals. That decision is made with your doctor or surgeon.

The usual phases

  1. Early recovery. Control swelling, regain full knee straightening, get the thigh muscles working and walk normally.
  2. Strength and control. Build leg strength, balance and single-leg control.
  3. Running and jumping. Start straight-line running, then landing and jumping drills, once strength and swelling allow.
  4. Sport-specific work. Cutting, pivoting, reacting and non-contact drills for your sport.
  5. Return to practice, then competition. Gradual exposure, from controlled practice to full games.

Your surgeon’s protocol sets the timing of each phase, especially early on.

What “ready” means

  • No swelling or giving way with training
  • Full range of motion
  • Thigh and hamstring strength close to the other leg
  • Hop and jump test results close to the other leg, with good landing control
  • Sport-specific training completed without problems
  • You feel confident in the knee
  • Clearance from your surgeon

Why not go back sooner?

Returning to pivoting sport early, before strength and control are restored, is linked with a higher risk of injuring the knee again. Young athletes are at the highest risk, which is why rushing a season rarely pays off.

Get medical attention if

After surgery you develop a fever, increasing redness or drainage from the incision, or a hot, swollen, painful calf. Go to emergency for chest pain or shortness of breath.
